Office Directions

Barcelona Counseling is located at 10445 SW Canyon Rd, #119B in Beaverton, OR 97005. The office is on the first floor, at the end of the hall. The main entrance and parking is on 106 St, across the street from the Enterprise Car Rental. The name of the building is Canyon Building.

 

New clients: Requirements

Before you make an appointment, it is required that you contact your health insurance and ask about your mental health behavioral benefits. It is important to know if you have any deductibles that have not been met, and if you have any copayments for your mental health counseling.

 

It is also required that you fill out all the documents needed for treatment. You can fill out all the forms through this website without having to print them. Please go to the Documents Section on this webpage. Additionally, it is also required that you  bring an ID and your health insurance card on the first visit.

 

Documentation needed for the 1st session

Oregon Health Share Clients

In order to use Care Oregon and Oregon Health Share, a pre- authorization is needed. The Oregon Health Plan might take up to 14 days to approve your authorization. You will be able to make an appointment once the pre-authorization has been approved.

Methods of Payments accepted

Your fee or co-payment will be collected at the time of services (not applicable for EAP). You can pay using your credit or debit card; however, a 3% transaction fee will be charged. To avoid the fee, you can also pay with cash, check, or Google Pay app.

Cancellations Policy

You will need to notify at least 24 hours in advance of any non-emergency cancellation. Failure to do so will result in a no-show fee that is a $50 or a use of your EAP approved voucher if applicable. Please note that no-shows are not the responsibility of your insurance company. A no-show will also result of a fee charge of $50.00.

 

Be aware that 2 no shows and/or 2 late cancellations (or the combination of both) will result in closing services for you. You will not be rescheduled if your first visit is a no-show. I will cancel future appointments if you are no longer eligible for benefits through current insurance. We may provide transitional appointments and include paying cash for services or free/reduced cost services elsewhere.

 

Terminating services

If we have a no-show from you, and/or you do not schedule an appointment after 30 days, we will assume you are ending your treatment. We may close your file at that time. You will be considered a new client after that period of time (30 days). You will not be again scheduled if the providers are full and/or is not possible to offer you regular ongoing appointments for treatment.

Telemedicine or Phone Sessions

We are currently offering video therapy or phone sessions at your request. Also offering Remote EMDR Video therapy.

All documents under the Documents Section will be needed before the first session. Additionally, it is needed a copy of your insurance card, and your Identification Document (Driver License, ID, and/or Passport).
